Update Block Notification Activity Report to include SSN, State and Zip

Currently, the SSN, State and Zip fields are not included on the Block Notification Activity Report as they were for Legacy. This request would include them in the Block Notification Activity Report going forward.

e-OSCAR NOTE: Will not include SSN Data in Activity Report.

OLDE Business Decision - will never include full SSN details within any Activity Report.

Through discussions with a cohort of Data Furnishers, determined limited value in including consumer State & Zip Code on any Activity Report.
