Add Company Info to Archive Extract Screen

Currently, the Archive Schedule page displays company, request and requestor information on the page where the company selects an Archive report, but this is not displayed on the Archive Extract page. This request would display the company information on the Archive Extract page similar to how it is displayed on the Schedule Archive page.
